I hereby express my willingness to enrol with the above-mentioned OSOC Trust as a Office Bearer, and undertake to attend all events organised by the Trust as and when directed, in accordance with its guidelines. Having read and understood the rules and regulations of the Trust, I agree to abide by them in letter and spirit, and shall duly notify the Trust in the event of my withdrawal from membership. I solemnly declare that no criminal cases or proceedings are pending against me as of this date, that all information stated herein is true and accurate to the best of my knowledge, and that I shall promptly inform the Trust of any changes to my personal details or credentials. I acknowledge that the Trust reserves the right to take appropriate action against me should any of the above facts prove to be false or misrepresented, and I respectfully request that my name be registered in its records.
